NOTARIZED CONSENT FOR TATTOOING OF A MINOR CHILD
This form grants legal permission for a minor to receive a tattoo. Must be notarized and submitted before the session. Mandatory by the State of Florida Department of Health.
NOTARIZED CONSENT FOR BODY PIERCING OF A MINOR CHILD
This notarized form authorizes body piercing services for minors with parental or guardian consent. Required by the State of Florida Department of Health.
